This internship learning contract will allow students to identify learning objectives they hope to meet while completing this internship.
Notes on completing this form:
- This learning contract must be completed and signed accordingly before registering for internship credit.
- This learning contract must be submitted to Registrar BEFORE beginning your internship.
- Each student will need to have this form approved and signed by: Faculty Internship Advisor, Academic Department Chair, the Student, the Internship Site Supervisor, and a representative of the Career Services Center. (Please bring it to the Career Services Center after collecting all the other signatures.)
