This learning contract must be completed and signed accordingly before registering for internship credit.
This learning contract must be submitted to Registrar BEFORE beginning your internship.
Each student will need to have this form approved and signed by: Faculty Internship Advisor, Academic Department Chair, the Student, the Internship Site Supervisor, and a representative of the Career Services Center. (Please bring it to the Career Services Center after collecting all the other signatures.)
