What do I do with my Admission Medical History & Immunization Forms?

Completed form must be turned in to the Student Health Center by the first day of class

• This form should be as complete as possible. Your health history is very important information which our health care providers will need to have if you need to be seen and treated at Student Health
• This form must be signed by parent/guardian if student under 18 years of age
• Immunization history must be completed
• Insurance information must be complete. You do not need to have medical insurance to be seen at Student Health, however we will need this information if lab work or diagnostic testing should be required.
