These are not online forms to fill out online rather they are forms you can print and use or mail in for action by the registrar. Transcript Form To request a transcript THIS FORM MUST BE SIGNED
Change of Degree Form To change your current declaration of degree
Declaration of Degree Form To change area of concentration; may be needed with change of degree application
Withdrawal Form To formally withdrawal from seminary.
I/E Form To request a course incomplete or extension - to be submitted to course instructor

Request for Directed Study or Independent Study

To request approval for an independent or directed study. A directed study is designed to allow students to pursue a course offering listed in the catalog without requiring attendance in regular class sessions. An independent study is designed to cover specialized material not usually included in a course offering listed in the catalog.
