Spring 2012 Online Evaluation Surveys for Students and Mentor Teachers -- Ashland Campus

Welcome to our "online evaluation survey" page. Below you will find the different evaluation forms for students and cooperating teachers to fill out. Locate the appropriate form and click on the site located with that form. The survey will walk you through each question (2 pages) and you must click on the Submit button at the end. All evaluations are anonymous and will be tabulated at the end of the semester and given to the University Supervisors for their review.

It should only take about 10 minutes to complete the evaluation/survey. We appreciate you taking the time to complete this evaluation/survey on your University Supervisor. These data will assist the university faculty/supervisors in their professional development.
