WHAT is Well Aware? This is a unique opportunity for employees to participate in various health screenings in a convenient, on-campus setting. Health personnel will complete a brief finger prick. Equipment will be on-site to identify your cholesterol, lipid profile and blood sugar levels. You will also identify your blood pressure, height/weight and body fat analysis. Finally, this data will be used to complete a personal and confidential health assessment questionnaire to further advance your health.
WHERE: The screenings will be held on Ashland University’s main campus in the Rybolt Sport Sciences Center (RSSC) in and around the Messerly Human Performance Lab (MHPL) across from RSSC 230. Signage will be present upon arrival.
WHO is eligible? Due to time and resource restrictions, this program is open to full-time faculty and staff. Eligibility for part-time employees and spouses on the health plan will be based upon the number of slots remaining on April 8. Part-time employees and spouses should call HR after this date for availability.
Is Well Aware mandatory? No, participation is completely voluntary. Employees participating in AU’s High Deductible Health Care Plan for the 2015-2016 plan year and wishing to receive HSA funding from AU, if available, must complete a physical at some point in 2015. This screening is an option to fulfill the physical requirement.
WHEN are the Well Aware screenings? Monday, April 13 and Tuesday, April 14 from 6:30am-9:30am. Sign-ups will be available every half-hour. Individuals are encouraged to reserve 45 minutes in their schedule to complete the entire screening. Attendees must be on-time.
Is there a cost? No! This program is being brought to you free of charge compliments of Ashland University and the EagleWell Program.
HOW do I sign-up? Participants must sign up in advance. Check-in will be required. There are around 120 slots during this initial opportunity to be filled on a first come/first sign-up basis. Human Resources is coordinating the sign-up process. Please contact HR at 419-289-5033 to arrange your date and time.
Must I “FAST” before my Health Screening? YES. What is “fasting”? Fasting is not eating or drinking for 12 hours prior to the screening- ONLY plain water is allowed. Otherwise, results may not be accurate.
HOW LONG will it take? Please reserve 45 minutes to complete the entire process.
What do I bring with me? Bring your AU ID. You do not need to bring money or anything else. There is no cost to participate.
HOW do I get my results? You will get all of your results immediately, with an opportunity to speak individually and privately with health personnel with any questions.
Is the program confidential? Yes, aside from the volunteers assisting in the screening, your information will not be shared and the results will have no impact on anything related to your employment.
