The mission of Ashland University Professional Development Services is to support and provide high quality professional learning opportunities that integrate 21st Century skills and incorporate high expectations for educators and other professionals.
Effective February 1, 2013, there will be 2.7% non-refundable service fee on credit card payments for tuition. This includes credit card payments made online, in-person and over the phone. The service charge will show as a separate transaction on your credit card statement. Electronic check transactions made online by entering an account number and routing number will be charged a non-refundable service fee of $2.95 per transaction.

Please note that the proposal form must be downloaded to your computer before it becomes fill-able. Once completed, save the file on your computer, then attach to an email to the Massillon-Stark Center.
Submit the following information to become an Ashland University PDS adjunct instructor
Photocopy of Teaching Certificate/License
Photocopy of Master’s Degree Grade Transcript
Tax and Employment Eligibility Forms (needed ONLY if you will be receiving an instructional fee from Ashland University)
- Ohio Employee’s Withholding Exemption Certificate (IT-4)
- Federal Employee’s Withholding Allowance Certificate (W-4)
- U.S. Department of Justice Employment Eligibility Verification (I-9)
- Direct Deposit Form
