Memberships can be purchased in the Rec Services Office or at the Customer Service desk. Two forms must be completed when purchasing a membership: a membership application and a liability form.
For community memberships - please allow 2 weeks for a background check to be cleared before admittance is granted
Gold Membership information (membership application, liability form, and FAQ's) can be found on the Employee Portal under Employee Wellness.
Forms may be typed (fillable pdf) or hand written and be:
- Faxed in to 419.207.6170 (x6170 on campus)
- Scanned and e-mailed to email@example.com
- Turned in to Rec Services at the Customer Service Desk
