Madison fellows should complete the Payment Request Form as completely as possible. Payment Request Forms must be submitted to the MAHG office electronically through the secure Adobe Sign form. Certain fields on the form that are specific to Ashland University and the MAHG and MASTAHG programs are prefilled for you.

Follow these directions for completing the remaining fields:

  • Number 1, enter your full legal name.
  • For "Subject of Degree Sought" (number 5) enter the program in which you are enrolled, either "American History and Government" for MAHG or "Teaching American History and Government" for MASTAHG.
  • Number 6, enter the option that applies to the program in which you are enrolled: 32 credit hours (MAHG) if you are on track to graduate by August 2026; 30 hours for MAHG (those graduating December 2026 or later); or 33 hours (MASTAHG).
  • Number 8, enter the year your fellowship was awarded.
  • Number 9, enter the number of hours approved for reimbursement from your Plan of Study: generally, 32 hours (MAHG prior to Fall 2026); 30 hours (MAHG students starting Fall 2026 or later); or 33 hours (MASTAHG).
  • Number 10, enter the number of this payment request (i.e. if this is the first time you've requested funds, enter 1; the second time, enter 2; etc.).
  • Number 11, enter the years for the academic year in which your course falls. An academic year is made up of a Fall, a Spring and a Summer semester in that order. Thus, Fall 2026 is in the 2026-2027 academic year, whereas Summer 2026 is in the 2025-2026 academic year.
  • Number 12, enter your current complete mailing address, email address, phone number and fax number (if applicable).
  • Number 13, the starting date of the term is the first day of the semester during which the course(s) will be held, even if the course only runs for a portion of the semester. See the Ashland University Registrar's Academic Calendar for the official semester start dates.
  • For summer courses in which you will reside on campus, enter your initials in the box to certify that it is necessary to live away from home. For fall and spring, or for summer terms in which you will not reside on campus, leave blank.
  • Number 14, line A, enter the amount of the tuition. Tuition rates vary by semester, course and location. Please see the Tuition and Fees page for current rates. You can retrieve the exact amount owed in Self-Service.
  • Number 14, line B, enter $0 in most cases. All fees are included with the tuition EXCEPT for the graduation fee. The $100 graduation fee is only assessed in your final semester at the time you apply for graduation with the AU registrar's office.
  • Number 14, line C, enter the actual cost of your books, if known. If you are submitting your Payment Request Form prior to the availability of the course syllabus, enter a reasonable estimate for book costs. Book costs for a typical course are no more than $75.
  • Number 14, line D, enter the amount of room and board cost (number of courses multiplied by this year's room and board rate). If you are NOT staying on campus in Ashland residence halls, enter the cost of your off-campus room (if necessary). Madison will reimburse up to the cost of the double-occupancy apartment rate. Fellows are responsible for early arrival, late departure, or the additional cost of a single room above the double-occupancy rate.
  • Number 14, line E, leave this field blank. If you are residing on campus (board costs are included in the Room figure on line D).
  • Number 14, line F, the total will be calculated for you automatically.
  • Number 17, the number of hours for full-time status is 6.
  • Number 18, enter the number of credit hours for which you are registered in this semester.
  • Number 19, enter the course codes of the classes for which you are registered this summer. For example, "HIST 501" or "POLSC 502".
  • In Number 20, you will report any outside financial aid you will receive other than your Madison Fellowship. This includes any scholarship aid from Ashland, tuition reimbursement from your employer and any other grant, loan, fellowship, or scholarship for which you are eligible and which you will receive for the courses listed on this PRF. This includes the First Course Waiver from Ashland for your first on campus course.
    • For Source of Grant or Loan, enter the entity that will award/has awarded you the funds. For aid received from Ashland, enter "Ashland University".
    • For Purpose of Grant or Loan, enter why you are receiving it (e.g. school district tuition reimbursement; student loan; etc.). For the Ashland first course waiver, enter First Course Tuition Waiver.
    • For Amount, enter the amount you will receive. For the first course waiver, enter the current cost of tuition (only) for one on-campus HIST or POLSC course.
    • If you are eligible for tuition reimbursement after completion of courses, reach out to the Madison office to discuss how they prefer to account for this.
  • Number 22, initial here only if you are NOT receiving outside aid (i.e. only initial here if you have left Number 20 blank.

Signing your PRF

On page 2, click the "Click here to sign" box and follow the directions to sign your name to the form.

Once you have signed, a button labeled "Click to Sign" will appear at the bottom of the screen. Click it to submit. Enter your email address, if prompted, then press "click to sign". Use of your ashland.edu email address is preferred.

You will then receive an email with a link to confirm the validity of your signature (thus our preference that you use your ashland.edu address).

Your form will be forwarded to the MAHG office for approval automatically.

Once the MAHG office has reviewed and approved your form, it will be forwarded on to the Madison office for you (2025 Fellows or earlier) OR it will be returned to you so that you may submit it yourself (2026 Fellows or after). A copy of the form will be kept on file at the MAHG office. You will also receive an email notice informing you that the form has been signed.
